I would go with just one map, since we'd need custom spawn points and it's a ranked event, throwing multiple maps in the mix could be considered unfair to some.

+ Not counting ranks if there are below 4 people on?

My few cents, for what they're worth:

Have it only run for one day at a specific time that is best for anybody. Say, a four hour event on Saturday. The winner shouldn't simply be the person that has played the most over an extended period of time, mainly because multi-day events don't drive sharp population. A one day event should see a full server; first come first serve, they know the date, they know the time, be there or be square.
No AWP/Auto. It's fine in 5v5 competitive, but it's a very cheap weapon for what should be a community event. AWP's also drive away non-Purist CS:S players, dying over and over isn't fun for them and will likely lead to a server full of AWP'ers, which isn't really that fun for anybody is it?
Multiple maps might be best if it's a one day event as I've suggested, just to avoid the population dropping via people getting fed up of the aforementioned de_season. 15 minutes per map sounds reasonable to me, no map voting or RTV's though and the map pool should be decided via a community vote to avoid terrible maps like de_piranesi (the worst map in the history of CS, even worse than de_tides).
I would recommend having a large array of prizes; maybe one for most kills, one for best KDR, maybe one for most knife kills too? Just to broaden the field and reward more players for their participation :)

The server will have 32 usable slots, and the event would last a full weekend.

- - What Prizes would you want to see for this event?: Mostly steam games motivate the people to play. Some of us play cs:go and there is some guns with skin and/or knives people might enjoy as a prize, maybe if at all possible, you can put some up for grabs?

- What maps would you like in this event's fixed rotation?: all of them! ( the standard maps coming with the game)

- What weekend (of this month) would fit best in your schedule?: 24-26 June that's next weekend
